Part 1: Filling Out the Application Form

Basic Personal Information (Fields 1-9)

  • Fields 1-3: Enter your name exactly as it appears in your passport

  • Field 4: Date of birth in DD-MM-YYYY format

  • Fields 5-7: Place and country of birth, current nationality

  • Field 9: Mark your current civil status

  • Field 11: National ID number (if applicable)

  • Fields 12-16: Passport information

    • Ensure passport is valid for at least 6 months beyond intended stay

    • Include issue date, expiry date, and issuing country

Address & Employment (Fields 19-22)

  • Field 19: Current residence address, email, and phone number

  • Field 20: If you have residency in another country, provide details

  • Field 21: Current occupation

  • Field 22: Employer details or educational institution information

Purpose & Duration (Fields 23-27)

  • Field 23: For D7, typically select "Other" and specify "Passive Income Visa/D7"

  • Field 24: Briefly explain your intention to relocate to Portugal

  • Field 25-26: Enter "PORTUGAL" as the destination

  • Field 27: Select "Two entries (residency)"

  • Include planned arrival date and duration of stay

Accommodation & Financial Information (Fields 30-32)

  • Fields 30-31: Provide Portuguese accommodation details

  • Field 32: Mark "by the applicant himself/herself" and indicate means of support

Part 2: Required Supporting Documents

  1. Passport (valid for at least 6 months)

  2. Financial Documentation:

    • Bank statements (last 6 months)

    • Proof of passive income

    • Investment statements

    • Minimum recommended amount: €760/month for the primary applicant

  3. Accommodation Proof:

    • Rental contract or

    • Property deed or

    • Letter of invitation with host's property documentation

  4. Criminal Record Check:

    • From the current country of residence

    • Must be recent (usually within 90 days)
